What should an SOP include?

A title and document control (owner, approver, version and review date), its purpose and scope, who is responsible for what, any definitions, what you need, the numbered steps, the records it produces and its revision history. This template has a section for each.

How long should an SOP be?

As short as it can be while someone new can still follow it without asking. Most SOPs for one task fit on one or two pages. If it runs longer, split it into separate procedures or move background into a policy.

Who should write an SOP?

The person who does the task best, working with whoever owns the process. A manager approves it, and a new starter tests it by following it.
