What should be included in an aged care shift handover?

Changes in each person’s condition, incidents and near misses, people at high risk, medication given, missed or changed, clinical tasks due, appointments and visitors, equipment problems, and any concern that was escalated.

Should handovers be verbal or written?

Both work best together. A face-to-face handover lets the incoming team ask questions, and a written sheet or record makes sure nothing is missed and shows what was passed on.

How long should a shift handover take?

Usually 10 to 20 minutes for a residential unit, depending on how many people and how much has changed. Focus on changes and risks rather than repeating everything for everyone.
